Sako <br />Deputy Director <br />—c <br />N <br />t—r <br />ry <br />rni <br />'. <br />t ,t <br />Re: Operating Budget <br />The Hawaii County Office of Aging recently partnered with the State Executive Office on Aging in <br />applying for the following federal grants: 1) Hospital Discharge Planning Model grant to develop a <br />model to put the patient and caregiver(s) at the center of the discharge planning process and <br />focus on discharging patients home with community -based services, via the Aging and Disability <br />Resource Center (ADRC), thereby reducing the number of patients retained in acute care beds <br />past the point of clinical discharge, and reduce the number of default discharges from acute care <br />units to nursing facilities; and 2) ADRC Expansion - Establish new or significantly strengthen <br />existing ADRC programs, develop a 5 year operational plan and budget. <br />The County was awarded $217,922 of the federal funding for these 2 pilot programs, over a <br />three year period. In addition, the State is offering the counties an additional $50,000 in state <br />general funds for ADRC operations. <br />Enclosed is a resolution authorizing the Mayor to enter into an agreement with the State of <br />Hawai'i Department of Health, Executive Office on Aging for the combined funding of $267,922. <br />Enclosed is a bill for an ordinance amending the Operating Budget by appropriating these grant <br />monies into the Area Plan on Aging account. <br />In order to receive the additional state funding of $50,000, the contract must be processed and <br />returned to the State no later than June 10, 2010. To meet this deadline, we request to waive <br />the Finance Committee and to have the resolution and bill heard at the Council Meeting <br />scheduled for June 7, 2010. <br />Comm.
