STATE OF HAWAII <br />SCOPE OF SERVICES <br />Attachment - S1 <br />8. Reporting Requirements. <br />a. Submit implementation data to EOA as defined by EOA for reports to Center for <br />Medicaid Services. <br />b. Submit in the format required by EOA a semi - annual ADRC Progress Report. <br />c. Submit in the format required by EOA a final ADRC Progress Report. <br />9. Comply with and implement the standards, best practices, protocols, models, evaluation, and <br />quality assurance that the task groups develop. <br />10. Collaborate with the EOA and its awarded consultant, HCBS Strategies, Incorporated throughout <br />the duration of this Contract.
