Provide options counseling training to staff on specific aging and disability topics (e.g., <br />private pay services, care home placement, disability services); <br />d. Make enhancements to the Hawaii ADRC website to improve access to inforruation and <br />services; and <br />e. Maintain an integrated data collection and reporting system for quality assurance and <br />evaluation. <br />The following service deliverables shall be met through collaboration with the STATE, Executive Office <br />on Aging ( "EOA "). <br />1. Delegate Area Agencies on Aging ( "AAA ") representatives and stakeholders to participate in <br />workgroups. <br />2. Participate in workgroups convened by BOA and HCBS Strategies Incorporated on the <br />following: <br />a. Participate in the development of the five (5) year Operational Plan and Budget for <br />achieving statewide coverage of Hawaii's ADRC. <br />b. Develop an eighteen (18) month timeline to develop the five (5) year ADRC Operational <br />Plan and Budget. <br />c. Work on the development of the HDPM. <br />i. Review hospital discharge planning protocols. <br />ii. Establish protocols for receiving referrals. <br />3. Participate in the Follow Up Conference in April 2011. <br />a.
