AGING <br /> AGING <br /> Program Objectives <br /> Non-Contracted Services <br /> 1. Aging and Disability Resource Center(ADRC) Development Project <br /> Provide older adults and caregivers information and assistance services. <br /> a. Provide quarterly activities marketing ADRC services using community <br /> presentations and resource fairs, and other media. <br /> b. Collaborate with tenant agencies and community partners to provide quarterly in- <br /> service trainings opportunities for ADRC tenant staff. <br /> c. Provide opportunity for AIRS training and certification to staff located in the <br /> ADRC who provide resource information to ADRC target population. <br /> d. Develop a Memorandum of Agreement with at least one acute hospital facility to <br /> streamline ADRC referrals and service access for eligible individuals. <br /> e. Develop a working plan for updating resource database and expand use of Social <br /> Assistance and Management System program software capacity to include Aging <br /> and Disability Resource Center service delivery documentation. <br /> 2. Caregiver Support: National Family Caregiver Support Program <br /> a. Develop and maintain information and outreach displays and presentations about <br /> the availability of support services in the community through public education in <br /> group settings (health fairs, community service fairs, or community organization <br /> meetings). <br /> • A minimum of one presentation will be given annually in each of <br /> nine districts island-wide. <br /> • HCOA will participate in at least two health and/or resource fairs, <br /> one in East Hawai'i and one in West Hawai'i. <br /> b. Provide access to all districts of the island, trained individuals to assess caregiver <br /> needs,provide information and assistance that link caregivers to needed services <br /> and provide follow-up on an as-needed basis. <br /> At least 80% of requests for information and/or services will have immediate <br /> (within 7 days) contact with an assessor to begin the assessment process and <br /> identify caregiver needs. <br /> c. Provide respite, support group, and counseling services for caregivers based on <br /> needs identified through the Montgomery-Borgatta Burden Scale screening tool. <br /> Respite services will include adult day care,personal care, and homemaker <br /> services. <br /> At least 80% of caregivers with needs identified will receive respite, support <br /> group, or counseling services. <br /> 4 <br />
