AGING <br /> AGING <br /> Program Objectives (continued) <br /> 3. Community Planning and Development—Partnerships <br /> Healthy Aging Initiative <br /> a. Increase by 2 the number of public and private agencies that will be members of <br /> the Hawai`i County Healthy Aging Partnership coalition. The increase would be <br /> based on the number of partners at the end of fiscal year ending June 2012. The <br /> intent is to embed a health related evidence-based program in Hawai`i County by <br /> building capacity and supporting healthy aging activities. <br /> b. Support and coordinate the Better Choices, Better Health—Ke Ola Pono Program, <br /> a chronic disease self management evidence-based community program. Deliver <br /> a minimum of 10 workshops serving 100 participants. <br /> 4. Education/Training <br /> Support and coordinate a minimum of two (2)workshops and/or educational seminars <br /> for Aging and Disability Resource Center(ADRC) clients and/or service providers <br /> that reflect identified needs of clients and providers. <br /> 5. Home Modification <br /> Enable older adults to live safely within their own homes, reducing caregiver stress <br /> and reducing falls in the home by correcting safety deficits. <br /> At least 75% of the clients served will report that the home modification items <br /> allowed them to remain in the home by increasing safety, reducing caregiver <br /> stress, and reducing falls in the home. <br /> Services Contracted to Private/State Agencies <br /> 1. Kupuna Care Program <br /> Case Management and Home and Community Based Services <br /> Kupuna Care Program services include case management, adult day care,personal <br /> care, home-delivered meals, heavy chore, homemaker, assisted transportation, and <br /> Kupuna Care transportation. <br /> a. Empower clients and/or caregivers through participation in service decision <br /> making. <br /> At least 90% of clients and/or caregivers will participate in care/service plan <br /> decision making. <br /> b. Enable older adults to remain in their homes for as long as possible through the <br /> provision of home and community-based services, thus preventing and/or <br /> delaying the need for institutional care. <br /> At least 70% of clients will remain in the home after three months of Kupuna <br /> Care home and community-based services. <br /> 5 <br />
