Provide HCOA staff access to training for AIRS certification. One hundred <br /> percent of HCOA ADRC intake staff will be AIRS certified. <br /> Alliance of Information and Referral Systems (AIRS) certification training <br /> will start Jan. 11,2012. The target for a Hilo certification test date is April <br /> 25,2012. HCOA's Aging and Disability Resource Specialist is AIRS <br /> certified. The Aging and Disability Resource Center Coordinator, the only <br /> other present intake staff,is scheduled for certification testing. Additionally, <br /> clerical support staff that answer agency telephones will participate in the <br /> AIRS trainings sessions. <br /> d. Develop a Memorandum of Agreement with at least one acute hospital facility to <br /> streamline ADRC referrals and service access for eligible individuals. <br /> The Hospital Discharge Planning grant implementation was again delayed <br /> related to Care Transition Intervention (CTI) training. Training on this <br /> model is projected for March 2012. A Memorandum of Agreement will be <br /> developed with Hilo Medical Center as the requirements of this model are <br /> clarified. <br /> e. Implement SAMS/IR as the data and_tracking software for all I&A contacts with <br /> the ADRC. <br /> The Social Administration Management System/Information & Referral <br /> (SAMS/I&R) data and tracking software are fully implemented. Hawaii <br /> County Office of Aging transitioned to an updated web based version of <br /> SAMS/I&R in November 2011. <br /> 2. Caregiver Support: National Family Caregiver Support Program <br /> a. Develop and maintain information and outreach displays and presentations about <br /> the availability of support services in the community through public education in <br /> group settings (health fairs, community service fairs, or community organization <br /> meetings). <br /> • A minimum of one presentation will be given annually in each of nine <br /> districts island-wide. <br /> • HCOA will participate in at least two health and/or resource fairs, one in East <br /> Hawaii and one in West Hawaii. <br /> Six Month Progress Report on Program Objectives for FY2011-12 2 <br />
