AGING <br /> AGING <br /> During the first six months of FY12, HCOA coordinated 2 caregiver <br /> conferences through the National Family Caregiver Support Program, <br /> "Caring for Family, Caring for Yourself"in Kona and "Share the Care"in <br /> Hilo. Each conference included keynote speakers, breakout presentations, <br /> and resource fairs which provided information on resources, programs, and <br /> services available for caregivers in Hawaii County. Presentation topics <br /> included: challenges of long-term care,protecting Medicare and Social <br /> Security, legal issues of caregiving,fall prevention, dementia and Alzheimer's <br /> disease, family dynamics in caregiving, emotional aspects of caregiving, brain <br /> health and research, fall prevention through Tai Chi, services for aging at <br /> home, talk story with the AARP National President, caregiver training and <br /> in-home tips,Medicare updates, and one-to-one Medicare enrollment <br /> assistance. The Hilo conference was held in collaboration with AARP, <br /> Project Dana, Hospice of Hilo,Alzheimer's Association, and the Honpa <br /> Hongwanji Hilo Betsuin Sanga Hall. The Kona conference was coordinated <br /> in partnership with AARP, the Executive Office on Aging SagePLUS <br /> program,the Department of Health,Alzheimer's Association, University of <br /> Hawaii Elderlaw Program, and Life Care Center of Kona. Attendance <br /> reached over 450 for the conferences with 300 participants in Hilo and 160 in <br /> Kona. The National Family Caregiver Support Program supported a <br /> Caregiver Training Workshop for family caregivers at the Life Care Center <br /> of Kona for 15 caregivers. Topics included: vitals, skin integrity, pain <br /> management, bathing tips, range of motion exercises, changing linens with <br /> care recipient in the bed, nutrition, hydration, quality of life, levels of <br /> activities, partnering with your doctor, dementia warning signs, aging <br /> process, community resources,Advanced Health Care Directives, body <br /> mechanics,transfers, and in-home safety. <br /> b. Provide access to all districts of the island, trained individuals to assess caregiver <br /> needs, provide information and assistance that link caregivers to needed services <br /> and provide follow-up on an as-needed basis. <br /> • At least 80% of requests for information and/or services will have immediate <br /> (within 7 days) contact with an assessor to begin the assessment process and <br /> identify caregiver needs. <br /> Six Month Progress Report on Program Objectives for FY2011-12 3 <br />
