AGING <br /> AGING <br /> b. Support and coordinate an evidence-based community program where at least <br /> 75%of participants surveyed will indicate that the evidence-based community <br /> program improved or maintained their health status. <br /> As part of the chronic disease self management program, each participant <br /> completes a follow-up survey 6 months after each workshop to check on the <br /> health status of the participants. For the first 6 months of FY 2012,3 <br /> workshops held their 6 month reunions; 27 participants completed the <br /> follow-up survey. All indicated improvement or maintenance of their health <br /> status. Examples of specific indicators were self-rated health, health distress, <br /> and such symptoms as fatigue, shortness of breath, and pain. Additional <br /> data was also collected including self-management behaviors, self-efficacy, <br /> and health care utilization, to name a few. An added bonus was that 3 <br /> participants experienced such a positive change in their lives,they continued <br /> on to become Lay Leaders. <br /> 4.
