AGING <br /> AGING <br /> The HCOA contracted with the non-profit organization, Services for Seniors, <br /> Inc.,through the National Family Caregiver Support Program to provide <br /> access and supplemental services for family caregivers. Caregiver <br /> assessments and care coordination services assist caregivers in obtaining <br /> access to Home and Community—Based Services which allow the family to <br /> remain in the home thus avoiding costly institutionalization. Access services <br /> include care coordination and Supplemental services include caregiver <br /> assessments. FY12 year to date services provided include: 402 units of access <br /> services to 79 family caregivers and 35 caregiver assessments for 35 family <br /> caregivers. An annual survey to collect caregiver satisfaction data will be <br /> completed by September 30, 2012. <br /> c. Provide respite, support group, and counseling services for caregivers based on <br /> needs identified through the Montgomery-Borgatta Burden Scale screening tool. <br /> Respite services will include adult day care, personal care, and homemaker <br /> services. <br /> • At least 80% of caregivers with needs identified will receive respite, support <br /> group, or counseling services. <br /> The HCOA contracted with 13 service providers through the National <br /> Family Caregiver Support Program to provide Home and Community— <br /> Based Services which include: respite care, counseling, and supplemental <br /> services. FY12 year-to-date services provided include: 2 family counseling <br /> sessions, 1,128 units of respite care for 46 family caregivers, and 80 units of <br /> supplemental services to 38 caregivers. An annual survey to collect caregiver <br /> satisfaction data will be completed by September 30,2012. <br /> 3. Community Planning and Development—Partnerships <br /> Healthy Aging Initiative <br /> a. Increase by 20%the number of chronic disease self management workshops <br /> offered in Hawai`i County. The 20% increase would be based on the number of <br /> workshops completed for the fiscal year ending June 2010. The intent is to <br /> embed a health related evidence-based program in Hawai`i County. <br /> For the FY ending June 2010, 10 self management workshops were <br /> completed. For the first 6 months of FY 2012, 3 chronic disease self <br /> management workshops were completed serving 21 individuals. The <br /> workshops were conducted in Waimea, Na`alehu, and Hamakua. During the <br /> same period, two Lay Leader trainings were conducted; graduating 13 new <br /> Lay Leaders certified in Chronic Disease Self Management. The trainings <br /> were conducted in Hilo and Kona.
