(Drug,Alcohol, and Tobacco)Awareness Program <br /> Project Sponsor: Hawaii Partnership to Prevent Underage Drinking (HPPUD) Statewide Coalition <br /> Project Manager: Jan Yokoyama <br /> Date: November 1, 2011 <br /> Purpose of the Teaming Agreement(TOA) <br /> This TOA serves as the guidelines and ground rules to help the project team work most productively together over the <br /> course of the project. The TOA is a living document and may be updated as needed. <br /> Groups must sign this TOA before receiving funds. This is a one-time, non-reimbursable award to support the <br /> coalitions, and should not be considered a long-term funding source. <br /> If there are any changes to your proposed project you must send in a revived proposal approved by the HPPUD <br /> Council Board Members. <br /> Project Overview <br /> County of Hawaii Liquor Control, D.A.T. (Drug,Alcohol,and Tobacco)Awareness Program,has been approved and <br /> awarded on the(7,11,11) in the amount of FIVE THOUSAND DOLLARS AND NO CENTS ($5,000.00). <br /> In 2011, the Hawai'i County D.A.T. (Drug, Alcohol and Tobacco)Awareness Program, under the direction of Jan <br /> Yokoyama and Iris McGuire, have trained 175 student-athletes from 14 private and public high schools throughout Hawai'i <br /> island . These student-athletes are committed to conduct Reach Out Now Teach-ins, a SAMHSA curriculum,for 5th and <br /> 6th graders in their feeder schools. The initial Teach-ins conducted in April and May reached 2644 students. Another <br /> round of Teach-ins will start in January 2012. This program is sponsored by the Hawai'i County Department of Liquor <br /> Control in collaboration with the Big Island Interscholastic Federation (BIIF). <br /> To assist with sustainability of this program, the student-athletes from Kea'au High School have expressed a desire to <br /> create a video called"If I Neva Do D.A.T." This video will be composed of a series of vignettes highlighting the <br /> consequences of using alcohol, tobacco and drugs. The student-athletes are creating and acting in the vignettes. Once <br /> the video is completed, the BIIF will be sharing it with all their participating schools, along with their respective feeder <br /> schools. <br /> The grant funds will be used to contract with daBraddahs who will provide direction to the student-athletes in creating and <br /> producing the video. <br /> Budget <br /> The awardees are to keep track and submit original receipts(not copies) of all funds used for this project to the <br /> HPPUD Statewide Coalition office for audit purposes. Please submit your first set of receipts February 1,2012 and <br /> remainder of your receipts March 31,2012.
