Please see attached OJD "Food&Beverage"guidelines. <br /> Funding will be disbursed through Hawaii County Liquor Control Office, approximately November 30, 2011, (pending <br /> original signature of this TOA)payable to:Dept. of Liquor Control. If this is incorrect you must immediately notify our <br /> office by telephone at 808.672.1517. <br /> According to the project financial budget the money will be spent as follows: <br /> DESCRIPTION AMOUNT <br /> Consultation fees with daBraddahs $5,000 <br /> Total:$5,000.00(total to include all fees, taxes, shipping <br /> and handling, not to exceed$5,000.00). <br /> These projects are to occur between November 1, 2011 to March 31, 2012. Any funds that has not been executed <br /> will be returned back to Hawaii Partnership to Prevent Underage Drinking(HPPUD)Statewide Coalition. <br /> Evaluation/Progress Reports <br /> You also agreed to provide two (2) reports which the second progress report will be your project evaluation report and <br /> final report. <br /> The first progress report is due February 1, 2012. Please create a document answering the following questions: <br /> 1. Please explain how you have engaged youth as leaders in the implementation of this project? <br /> 2. What steps in your proposal have you completed and what steps do you still need to complete? <br /> 3. Of the steps that you still need to complete, do you expect to finish them within your proposed timeline? If <br /> not,when do you expect to complete them? <br /> 4. How many meetings, events, etc., have occurred and how many people participated? <br /> *Please provide a full breakdown of how the budget was spent thus far. <br /> The final progress report and project evaluation is due March 31,2012. Please create a document answering the <br /> following questions: <br /> 1. How do you know that you were successful in completing your project? <br /> 2. How do you know that your project had an impact in preventing underage drinking, or in changing the <br /> awareness, attitudes, or behaviors related to underage drinking? <br /> 3. Are there any activities/steps that you didn't complete? If yes, please explain why. <br /> 4.
