Dear County Officials: <br /> The NACo Dental Discount Card Program contract is attached in a separate file. Please review the <br /> checklist in this document. Incorrect checklists or contracts will result in a delayed implementation of <br /> the program. <br /> Please follow these directions carefully: <br /> I. Counties,Parishes,Boroughs sign Schedule 4 of the contract which attaches the county to the <br /> overall NACo contract. <br /> 2. Once signed,mail the Schedule 4 page and the checklist to: <br /> Andrew S.Goldschmidt CAE,MBA <br /> Director,Membership Marketing <br /> NACo—National Association of Counties <br /> 25 Massachusetts Ave,NW,Ste. 500 <br /> Washington,DC 20001 <br /> 3. Once the contract is received,our executive director will sign it and pass it on to Careington for final <br /> signature.A fully executed copy will be returned to you. You may start your internal planning process to <br /> promote the program. <br /> 4.While the final signatures are being completed,please forward your logo or seal via e-mail to <br /> agoldschmidt@naco.org.
