Transportation <br /> Provide transportation service to congregate meals participants in order for them to <br /> receive nutritious meals and participate in site activities that enhances physical and <br /> social well-being. <br /> a. The percentage of participants identified as non-nutritional risk at the <br /> beginning of the program year shall be maintained at the same or higher levels <br /> by the end of the program year. <br /> b. The percentage of participants identified as high nutritional risk at the <br /> beginning of the program year shall be maintained at the same or lower levels <br /> by the end of the program year. <br /> The Office of Aging will proceed with projections based on Area Plan on Aging and <br /> State Plan on Aging goals and objectives and anticipated revenues. Due to an <br /> unforeseen financial situation at the federal and state levels, all plans, goals, and <br /> objectives are contingent upon receipt of federal, state and county funds as anticipated <br /> and these budget projections are subject to change. <br /> In the first five months (July 1 —November 30,2012) of FY 2012-2013, <br /> transportation contractor HCEOC provided 15,959 trips to a monthly average of <br /> 163 (unduplicated) seniors across Hawaii Island. Note: numbers for December <br /> 2012 are not available at this time. <br /> Transportation is an essential service for seniors that allows them to access <br /> activities such as medical visits, employment, grocery shopping and nutrition <br /> programs. Initial numbers indicate that HCEOC needs to increase their trips <br /> and unduplicated riders. HCOA staff has provided technical assistance to <br /> HCEOC; an outreach plan has been developed that includes collaboration with <br /> the Hawaii County Nutrition Program in outreach efforts to seniors. <br /> Contracts to the Elderly Activities Division,Department of Parks & Recreation. <br /> Funds provided to the Elderly Activities Division are deposited directly to its respective <br /> accounts.
