BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): <br /> Each year more than 1/3 of our population, age 65 and over, suffers a fall. These falls account for one out of <br /> every 10 emergency department visits in our country and over $20 billion in healthcare costs each year. <br /> Specifically within the Hawaii County, this equates to over 1,400 emergency department visits, over 400 <br /> hospital admissions, and seven deaths each year. <br /> In a partnership with Hilo Medical Center, State Emergency Medical Services and Injury Prevention, and <br /> Hawaii County Office of Aging (ADRC), the Hawaii Fire Department is preparing to rollout a Senior Fall <br /> Prevention and Home Safety Program in which EMS paramedics will perform a standardized fall risk <br /> assessment, complete a fall prevention home safety checklist, provide fall safety education, and when <br /> necessary make referrals within the community healthcare system to assure patient safety, prevent falls, and <br /> ultimately improve outcomes and quality of life for the Kupuna on Hawaii Island. <br /> This funding from Walmart will aid in the purchase of a laptop computer to assist in management and data <br /> collection for the EMS Bureau's Fall Prevention Program. <br /> SIGNED: DATE: /L/ii'�i <br /> Department Head <br />
