AGING <br /> AGING <br /> Empower older adults to stay healthy, active, and socially engaged using prevention <br /> and disease self-management techniques. <br /> 1. Healthy Aging Initiative <br /> a. Expand the Better Choices, Better Health—Ke Ola Pono Program, a chronic <br /> disease self-management evidence-based community program to the districts of <br /> Puna(Mauka and Makai) and Kohala(North and South). Deliver a minimum of 2 <br /> workshops serving 16 participants in each district. <br /> During the first 6 months of FY 2014, Office of Aging conducted one <br /> workshop at Tutu's House in Waimea. Participants numbered at eleven and <br /> all completed the Chronic Disease Self Management workshop. <br /> During this period all Master Trainers and Lay Leaders were concentrating <br /> on update training due to revisions in the Chronic Disease Self Management <br /> curriculum. After receiving updated training,Lay Leaders were gearing up <br /> for new workshops to start January 2014. <br /> b. Build capacity of the Better Choices, Better Health—Ke Ola Pono Program in the <br /> Hilo and Kona districts. Conduct Lay Leader training in each district <br /> graduating16 Lay Leaders. <br /> Three Big Island Master Trainers conducted Lay Leader update trainings in <br /> July and September. The training at Tutu's House in Waimea updated 5 <br /> Lay Leaders and the training at the ADRC in Hilo updated 8 Lay Leaders. <br /> Enable older adults to remain in their homes with a high quality of life for as long as <br /> possible through the provision of home and community based services, including <br /> supports for caregivers thus preventing and/or delaying the need for institutional care. <br /> 1. Kupuna Care Program Services. These Home and Community Based Services <br /> include case management, adult day care,personal care, home-delivered meals, <br /> heavy chore, homemaker, transportation, and home modification. <br /> a. A minimum of 600 older adults (unduplicated count) received home and <br /> community-based services through the Kupuna Care Program. As a result, 70% <br /> of clients will remain in the home after three months of Kupuna Care services. <br /> Six Month Progress Report on Program Objectives for FY 2013-14 1 <br />
