AGING <br /> AGING <br /> 291 older adults received home and community-based services through the <br /> Kupuna Care Program. Of the 104 discharged during this time period, 75% <br /> of clients remained in the home. <br /> b. A minimum of 125 older adults receive home modification services that enable <br /> them to live safely within their own homes. As a result, least 75% of the clients <br /> served will report that the home modification items allowed them to remain in <br /> their home by increasing safety, reducing caregiver stress, and reducing falls in <br /> the home. <br /> During the period from 07/01/13 to 12/31/13, the Home Modification <br /> Program provided 69 units of home modification items to improve in-home <br /> safety for 28 frail and vulnerable adults. An annual client satisfaction survey <br /> will be conducted in May,2014. <br /> 2. Caregiver Support: National Family Caregiver Support Program <br /> Trained individuals will assess caregiver needs, provide information and assistance <br /> that link caregivers to needed services, and provide follow-up as needed. <br /> a. A minimum of 100 caregivers receive information and assistance. As a result, at <br /> least 80% of requests for information and/or services will have immediate (within <br /> 7 days) contact with an assessor to begin the assessment process and identify <br /> caregiver needs. <br /> 84 caregivers received information and assistance. During the same period, <br /> 74 received an assessment. <br /> b. Based on needs identified a minimum of 100 caregivers are provided respite, <br /> support group, and counseling services. <br /> 117 caregivers were provided with these services. <br /> 3. Transportation to enable older adults to access community services by providing <br /> transportation to/from congregate meal sites, medical and health related visits, <br /> grocery shopping and other community resources. <br /> a. A minimum of 350 older adults received transportation services to congregate <br /> meal sites and other community resources. 80% are satisfied with the serviced <br /> received. <br /> 203 older adults received non-governmental organization contracted <br /> transportation services. <br /> Six Month Progress Report on Program Objectives for FY 2013-14 2 <br />
