AGING <br />AGING <br />Program Description (continued) <br />8. To maintain and foster collaborative relationships with relevant eldercare, <br />community based private and public agencies including those working with the <br />disabled population of all ages; <br />9. To promote new or expanded benefits and opportunities for older persons; <br />10. To develop and administer an Area Plan on Aging for a comprehensive and <br />coordinated service delivery system in Hawaii County; <br />11. To enter into sub -grants or contracts for the provision of services outlined in the <br />Area Plan on Aging; and <br />12. To provide technical assistance, monitor, and periodically evaluate the performance <br />of all service providers under the Area Plan on Aging. <br />Program Objectives <br />Maximizing opportunities for seniors to aging well, remain active, and enjoy quality <br />lives while engaging in their communities. <br />1. Community Planning and Development/Healthy Aging Initiative will continue to <br />sustain the Better Choices, Better Health — Ke Ola Pono Program, a chronic disease <br />self-management evidence -based community program. Office of Aging will continue <br />to build capacity by developing a marketing and recruitment plan to increase the <br />number of Lay Leaders. <br />a. Develop a marketing plan to recruit potential Lay Leaders by June 30, 2016. <br />b. Build capacity of the Better Choices, Better Health — Ke Ola Pono Program, by <br />conducting Lay Leader trainings in the Kona and Hilo districts. Conduct Lay Leader <br />trainings in each district graduating 12 Lay Leaders for the fiscal year ending June 30, <br />2016. <br />Enable older adults to remain in their homes with a high quality of life for as long as <br />possible through the provision of home and community based services, including <br />supports for caregivers thus preventing and/or delaying the need for institutional care. <br />
