AGING <br />AGING <br />Program Objectives (continued) <br />Kupuna Care Program Services. These Home and Community Based Services <br />include case management, adult day care, personal care, home -delivered meals, <br />heavy chore, homemaker, assisted transportation, and home modification. <br />a. A minimum of 500 older adults (unduplicated count) will receive home and <br />community-based services through the Kupuna Care Program. Upon discharge, <br />70% or more of clients will indicate they were able to remain in their home after <br />receiving Kupuna Care services. <br />b. A minimum of 100 older adults will receive home modification services to enable <br />them to live safely within their own homes. At least 75% of the clients served <br />will report the home modifications increased safety, reduced caregiver stress, and <br />reduced falls, allowing them to remain living in the home. <br />2. Caregiver Support: National Family Caregiver Support Program <br />Trained individuals will assess caregiver needs, provide information and assistance <br />that link caregivers to needed services, and provide follow-up as needed. <br />a. A minimum of 4 educational activities, reaching an estimated 100 caregivers <br />receive information and assistance. At least 80% will have an improved <br />knowledge base of caregiver resources to enable informed decision making. <br />b. Based on needs identified through access assistance, a minimum of 100 caregivers <br />will be provided with information, training, respite, support group, and counseling <br />services. A complete caregiver assessment will be provided to at least 75% of <br />these individuals. <br />Public Education will offer meaningful information about the services and benefits <br />available to older adults and their caregivers living in Hawai' i County. <br />a. A minimum of 4,200 householders, physician offices, and organizations receive <br />the monthly publication called the "Silver Bulletin," which aims to increase the <br />knowledge of and access to benefits and programs for older individuals and their <br />caregivers in Hawaii County. <br />M <br />
