AGING <br /> AGING <br /> Maximizing opportunities for seniors to age well, remain active, and enjoy quality lives <br /> while engaging in their communities. <br /> 1. Community Planning and Development/Healthy Aging Initiative will continue to <br /> sustain the Better Choices, Better Health—Ke Ola Pono Program, a chronic disease <br /> self-management evidence-based community program. The Hawai`i County Office <br /> of Aging (HCOA)will continue to build capacity by developing a marketing and <br /> recruitment plan to increase the number of Lay Leaders. <br /> a. Develop a marketing plan to recruit potential Lay Leaders by June 30, 2016. <br /> Marketing for Lay Leaders was done through HCOA's newsletter titled the <br /> "Silver Bulletin" and also in HCOA's mail outs titled "The Comfort of Home <br /> Caregiver Assistance newsletter". These newsletters and inserts increased <br /> awareness of the Better Choice Better Health Program throughout our <br /> community. Additionally,the Office of Aging strengthened our relationships <br /> with partners the Department of Health - Public Health Education Chronic <br /> Disease Management Branch on Hawaii Island, Tutu's House in Waimea <br /> and Miloli`i Community to educate more people about our program and <br /> need for Lay Leaders. Overall,these efforts helped us to recruit nine new <br /> potential Lay Leaders. <br /> b. Build capacity of the Better Choices, Better Health—Ke Ola Pono Program,by <br /> conducting Lay Leader trainings in the Kona and Hilo districts. Conduct Lay <br /> Leader trainings in each district graduating 12 Lay Leaders for the fiscal year <br /> ending June 30, 2016. <br /> A Chronic Disease Self-Management Program (CDSMP)Lay Leader <br /> Training took place on June 1,2, 8 and 9,2016 in Waimea at Tutu's House. <br /> Of the nine participants registered, seven completed the course. Two <br /> participants were from the National Kidney Foundation and another two <br /> were from the West Hawaii Community Health Center with the rest of the <br /> participants coming from both West and East Hawaii. <br /> Final Status Report on Program Objectives for FY 2015-16 1 <br />
