DPW/LRB CONSTRUCTION CODE - DRAFT 5/26/20 For review and comment Article 4. Permit Application. Division 1. Application. Section 5-4-1. Application for permit. (a) To obtain a permit, an applicant shall file an application in writing or online on a form furnished by the authority having jurisdiction for that purpose. Applications shall: (1) Identify and describe the work to be covered by the permit for which application is made and the owner of the property, {17-18(a).) (2) Describe the property on which the proposed work is to be done in sufficient detail to determine the precise location of the property involved, including legal description, tax map key number, street address, or similar description that will readily identify and definitely locate the proposed building or work; {5-20(2), 17-18(a).) (3) Indicate the use and occupancy for which the proposed work is intended; {5-20(3), 17-18(a).) (4) Be accompanied by plans, specifications, construction documents and other information as required in this article; {5-20(4), 17-18(a).) (5) If submitting plans, specifications, or construction documents pursuant to section 464-13(b), Hawaii Revised Statutes, the applicant shall include proof of recordation with the bureau of conveyances in accordance with section 464-13(c), Hawaii Revised Statutes. (6) State the estimated value of the proposed work; {5-20(5).) (7) Indicate that the owner or the owner's authorized agent consents to the permit application, by inclusion of the owner or the owner's authorized agent's signature on the application; {5-20(6).) (8) State the following information for each contractor or subcontractor engaged to do electrical or plumbing work upon the building, structure or project: (A) Name; (B) Name of the responsible managing employee or authorized employee of each contractor or subcontractor; (C) Address, email address and telephone number; (D) Contractor's license number; and (E) Particular phase or phases of work to be performed; (9) Provide the name and license number of all specialty contractors involved in the project, in compliance with the provisions of chapter 444, Hawaii Revised Statutes; (10) Be signed by the responsible managing employee or authorized employee of each contractor designated in paragraphs (8) and(9); (11) Provide email and telephone number of applicant or agent; (12) Include a non-refundable permit plan review fee pursuant to section 5-7-1; and {17-18(a).) (13) Contain any other information necessary to enforce the provisions of the construction code as may be required by the authority having jurisdiction. {9-30(a).) 18
