(1) Identify and describe the work to be covered by the permit for which the <br /> application is made and the owner of the property; <br /> (2) Describe the property on which the proposed work is to be done in sufficient <br /> detail to determine the precise location of the property involved, including legal <br /> description,tax map key number, street address, or similar description that will <br /> readily identify and definitely locate the proposed building or work; <br /> (3) Indicate the use and occupancy for which the proposed work is intended; <br /> (4) Be accompanied by plans, specifications, construction documents and other <br /> information as required in this article; <br /> (5) If submitting plans, specifications, or construction documents pursuant to section <br /> 464-13(b), Hawaii Revised Statutes, the applicant shall include proof of <br /> recordation with the bureau of conveyances in accordance with section 464-13(c), <br /> Hawaii Revised Statutes; <br /> (6) State the estimated value of the proposed work; <br /> (7) Indicate that the owner or the owner's authorized agent consents to the permit <br /> application, by inclusion of the owner or the owner's authorized agent's signature <br /> on the application; <br /> (8) Declare the phases of work, if any, that will be performed by a contractor or <br /> specialty contractor, or both, as required by law; <br /> (9) Provide the name and license number of all specialty contractors involved in the <br /> project, in compliance with the provisions of chapter 444, Hawaii Revised <br /> Statutes; <br /> (10) Be signed by the responsible managing employee or authorized employee of each <br /> contractor designated in paragraphs (8) and (9); <br /> (11) Provide email and telephone number of applicant or agent; <br /> (12) Include a non-refundable permit plan review fee pursuant to section 5-7-1; and <br /> (13) Contain any other information necessary to enforce the provisions of the <br /> construction code as may be required by the authority having jurisdiction. <br /> (b) Applications and required documents shall be filed in writing. After the authority <br /> having jurisdiction implements its electronic filing system, applicants may submit <br /> documents in writing or electronically. <br /> (c) Applications that do not conform to the provisions of division 2 shall not be accepted <br /> by the authority having jurisdiction. <br /> Section 5-4-2. Plans, specifications, and other data. <br /> (a) The following documents shall be submitted with each application for a permit: <br /> plans; specifications; engineering calculations; diagrams; soil investigation reports; <br /> code search; special inspection and structural observation programs; and other data as <br /> may be required by the authority having jurisdiction. <br /> 27 <br />
