Normal hours for <br /> appointments will be between 8:00 AM and 5:30 PM, Monday <br /> through Sunday. Normal business levels include 10-26 hours <br /> per week of appointments. <br /> 2) Proposed number of visits/clients per day: <br /> An average of 15 to 20 clients per day are anticipated with an <br /> expected maximum of 40 clients per day. <br /> 3) Proposed number of parking stalls: <br /> Four parking stalls plus 1 ADA compliant parking stall are <br /> planned for the clinic. <br /> 4) Proposed staff: <br /> Two to Three staff are anticipated. <br /> The construction of the first dwelling is anticipated be done by <br /> January 2020. The 21° dwelling is planned to be completed within <br /> two years after that, as budget allows. The estimated cost of <br /> construction of the first dwelling is $75,000 (excepting clearing, <br /> septic and catchment). The second dwelling will have a similar cost. <br /> This area is not served by a County sewer system so new <br /> construction will be required to provide an Individual Wastewater <br /> System meeting the requirements of the Department of Health <br /> designed by a civil engineer who is licensed by the State of Hawaii. <br /> 3B. PROJECT LOCATION <br /> As noted earlier, the subject site consists of an approximate 1-acre <br /> portion of the 2-acre parcel identified by TMK: (3) 1-6-009: 017.The <br /> subject site is located off of(lima Street, approximately .2 miles <br /> northwest of Ainaloa Boulevard, between 35th and 36Th Avenue in <br /> Orchidland Estates (Figure 1). <br /> The 2-acre parcel is rectangular shaped and is currently mostly <br /> covered in vegetation. The subject 1-acre portion of the parcel sits <br /> at approximately 525 ft of elevation. Presently there are no <br /> structures on the site. The remaining 1-acre, at the rear of the <br /> property will be used as the Applicants principal residence. <br /> 2 <br />
