The Personal History Questionnaire Review. <br />REP. SHOOK: Okay, Mr. Shook, please read the last paragraph that is stated there. <br />APPELLANT SHOOK: "In the summer of 2005, in Hilo, Hawaii at the intersection of Kilauea <br />Avenue and Mohouli Street, I was involved in a two -car accident. I rear ended a Toyota Camry <br />that was stopped at the intersection. I expected the Camry to drive through the intersection, the <br />Camry stopped and I was unable to stop in time." <br />REP. SHOOK: Mr. Shook, you got into an accident. Please tell them what your age was on <br />2005? <br />APPELLANT SHOOK: In 2005, at this time, I was I believe 14. <br />REP.
