SHOOK: It's on Exhibit 3. <br />CHR. NAMAHOE: All right. <br />REP. SHOOK: And it's under Communications 20-01-03, and it's Number 3. <br />December 16, 2020 <br />CHR. NAMAHOE: So, we're in Exhibit 3, there are four pages—would it be page 1, 2, 3, or 4? <br />REP. SHOOK: It would fall under Personal History Questionnaire Review. <br />MS. HUGHES: What page would that be? <br />REP. SHOOK: It doesn't have a page number. <br />CHR. NAMAHOE: Again, just— <br />REP. <br />ust <br />REP. SHOOK: It's just before—it's the page just before Exhibit 3 -just before Exhibit 3. <br />CHR. NAMAHOE: So, this is still Exhibit 2? <br />REP. SHOOK: That would be exhI'm sorry, they were just stuck together. <br />CHR. NAMAHOE: So, on page 5 of 5? Is that what you're saying? <br />REP. SHOOK: Yes. That would be the last page, just before Exhibit 3. <br />MR. CHILLINGWORTH: Madam Chair, Chillingworth speaking. I can't hear the statements <br />that are being made by Representative Shook and I'm going to ask him to get closer to the <br />microphone, please. <br />REP. SHOOK: I'm sorry. It's Exhibit 2, the exact last page. It's the Personal History <br />Questionnaire Review. It's the bottom of the statement, the very bottom of the statement, just <br />before the signature.
