EXAMPLE PROJECTS WHICH BEST ILLUSTRATE THE FIRM'S QUALIFICATIONS FOR THIS PROJECT CATEGORY 19.TITLE AND LOCATION (City and State) 20.YEAR COMPLETED Large Capacity Cesspool Statewide Audit for the State of PROFESSIONAL SERVICES CONSTRUCTION (If applicable) Hawaii-Oahu,Maui,Molokai,Kauai,Hawaii Island, 2023-Ongoing N/A Hawaii detailed recommended corrective actions, cost estimates for those actions, and a schedule of completion that complies with the CA/FO. Finally, individual site reports that document the absence of an LCC for the remaining thirty(30) subject leases were included. ➢ Phase II - Islands of Kauai, Maui,and Molokai: "EPA Docket No. UIC-09-2022 -006 Inspection Completion Report - Phase II (Kauai/Maui/Molokai)" dated February 2024 was submitted to the EPA. Thirty (30) subject leases were evaluated, some of which were comprised of multiple TMKs. Twenty (20) of the leases were on Kauai and ten (10) were on Maui.Three (3) of the Maui properties were located in the August 8-9, 2023 fire-impacted Lahaina area and, per EPA"Approval of Extension to Complete Phase II) dated October 13, 2023, submittal of inspection reports for these properties will be submitted by the extension deadline April 19, 2024. Therefore, of the thirty (30) subject leases evaluated, twenty-seven (27) leases are evaluated in the February 20204 report. Of those leases surveyed, three (3) were found to have LCCs. For two (2) of those properties with LCCs, the construction and installation of an Individual Wastewater System (IWS) was recommended. For one (1) property, the recommendation was to connect to the nearby existing sewer system. A general schedule for corrective actions was presented. Individual site reports for properties with an LCC were included, each of which provided details on recommended corrective actions, cost estimates for those actions, and a schedule of completion that complies with the CA/FO. Finally, individual site reports that document the absence of an LCC for the remaining twenty-three (23) subject leases were included. ➢ Phase Ila - Lahaina, Maui: "EPA Docket No. UIC-09-2022-006 Inspection Completion Report - Phase Ha (Lahaina, Maui)" was completed in April 2024. Three (3) subject leases were evaluated in Lahaina, Maui. These properties are located in the fire-impacted Lahaina area and, per EPA "Approval of Extension to Complete Phase II" dated October 13, 2023, submittal of inspection reports for these properties are being submitted in Phase Ila by the extension deadline of April 19, 2024. This report detailed the findings from those three (3) properties. Of those leases surveyed, two (2) were found to have LCCs. For the two (2) properties with LCCs, the construction and installation of an Individual Wastewater System (IWS) was recommended. A general schedule for corrective actions was presented. Finally, individual site reports that documented the absence of an LCC for the remaining one (1) subject lease was included. ➢ Phase III - Hawaii Island: E2 has completed the inspection validation fieldwork for 58 sites on Hawaii Island and is currently drafting the finalized Inspection Completion Report. The final report is expected to be submitted in 2025 to the DLNR and EPA. E2 will prepare closure reports for the LCCs that are closed by the tenants and DLNR. The closure reports will document that the LCCs were properly closed in accordance with EPA and DOH requirements. E2 will certify and sign each independent LCC closure report incorporating DOH permits to operate an IWS or approval to connect to a sewer, and copies of the EPA LCC Backfilling Final Completion Report or approved copy of the DOH UIC abandonment application.
