Takase <br />Director <br />SUBJECT: Application No. 25-06, Black Onyx Entertainment LLC dba Black Onyx, for an <br />original Cabaret liquor license, located at 202 Kamehameha Avenue, Hilo, Hawaii 96720; <br />TMK: 2-3-007:015. <br />NATURE OF APPLICATION: This is an application for an original Cabaret liquor license. <br />CLASS 11, CABARET LICENSE: A cabaret license shall be general only and shall authorize <br />the sale of liquors for consumption on the premises. This license shall be issued only for <br />premises where food is served, facilities for dancing by the patrons are provided, and live or <br />amplified recorded music or professional entertainment, is provided. <br />Meals prepared in suitable kitchen facilities connected therewith, containing the necessary <br />equipment and supplies for cooking an assortment of foods, shall be available to patrons while <br />the premises is open for business. The premises shall have a minimum of 100 square feet <br />suitable for dancing. Each area where dancing by patrons will be provided shall contain no less <br />than 100 square feet and shall not obstruct the normal flow of traffic. <br />APPLICANT: The applicant, Black Onyx Entertainment LLC, is a domestic limited liability <br />company. registered with the Department of Commerce and Consumer Affairs, State of Hawaii, <br />on August 29, 2024. It is manager -managed with its sole manager as follows: <br />Tasmine Curenton <br />The name of the registered agent in the State of Hawaii is as follows: <br />Tasmine Curenton, 202 Kamehameha Avenue, Hilo, HI 96720. <br />PERSONAL HISTORY: For information relative to the personal history of the applicant, <br />please refer to Exhibit 1 attached. <br />OWNERSHIP STRICTURE: Black Onyx Entertainment LLC is the operating entity that will <br />hold the liquor license.
