PART 9 APPEALS FROM AGENCY DETERMINATION <br />A person aggrieved by the determination of the Agency has a right <br />to request a contested case hearing. The request shall: <br />9.1 be filed within thirty (30) days after receiving notice of the <br />determination by the Agency. <br />9.2 be 'filed with the office of the Agency at 35 Wailuku Drive, Hilo, <br />Hawaii 96720. <br />9.3 conform to the general requirements of Rule 2.5 unless otherwise <br />specifically provided by a particular applicable rule. <br />9.4 the contested case hearing shall be conducted in accordance <br />with the Agency's Rule No. 5. <br />-16- <br />
