Coordinated Services for the Elderly <br /> <br />CSE & LOGISTICARE TRANSPORTATION SERVICES <br />CSE is now contracted with LogistiCare to provide non-emergency medical transportation services to <br />any Medicaid EverCare member. If you are a Medicaid recipient and have EverCare as your <br />provider and you need to arrange for transportation to a medical appointment just call LogistiCare <br />at 1-866-475-5746. This number is also listed on the back of your card. When calling they will ask you <br />for your full name, date of birth, EverCare member number, your home address, zip code and phone <br />number as well as the date, time and location of your medical appointment. Appointments must be <br />scheduled at least 48 hours in advance. Again, please request that CSE provide your transportation <br />services when calling LogistiCare. Mahalo! <br /> <br />CSE HELPING SENIORS WITH MEDICARE & MEDICAID ENROLLMENT <br />CSE has received a special grant to assist seniors to access low income subsidy assistance. Many <br />seniors qualify for help they are not currently receiving. If you are a senior whose income is below <br />$1,557.50 per month for a single person or below $2,095.00 per month for a married couple, please <br />contact CSE so our staff can make sure you are getting all the benefits you deserve. CSE staff can also <br />assist any senior with determining which Medicare health plan is best for you. Call the CSE I & A Hotline <br />at 961-8777. <br /> <br />FREE LEGAL HELP FOR SENIORS <br />Hawai‘i Seniors now have access to FREE legal assistance because of a special grant program with <br />Legal Aid Society of Hawai‘i. Seniors can get help with a wide variety of legal issues including <br />Advanced Health Care Directives, Power of Attorneys and Simple Wills. For more information about <br />these free services, please call toll free 1-888-536-0011. <br /> <br />CSE SPOTLIGHT KA‘U DISTRICT <br />Terry Saito has been with CSE for 10 years and loves working in the Ka‘u district. Terry explains, “the <br />seniors in Ka‘u are like family to me, they have adopted me and I am willing to do whatever I can to help <br />them.” Terry has offices in both Pāhala and in Nā‘ālehu. Most of the time you will find her working out of <br />the Pāhala office which is located right in the senior housing complex. <br />Sophia Iaukea has also helped out in the Ka‘u district for over 5 years. The seniors in the district know <br />her best as their “Sweet Van Driver” as she does the weekly transportation runs to Hilo every Wednesday <br />and Thursday. Sophia says she loves working in the Ka‘u district because, “all the people are so nice. <br />I was raised on the plantation so I like the country feel of Ka‘u. We all know how to get along.” <br />Terry and Sophia also keep busy assisting seniors with I & A services as well as partnering with the <br />Food Basket to offer qualifying seniors free produce in the summer and can goods in the fall as well as <br />federal surplus food 4 times a year. <br />Ka‘u is a huge district and with out the help of volunteers not everything could get done so well. <br />Larry Fisher is one of the districts special volunteers. For over 8 years he has been providing <br />transportation services for Ocean View seniors who need to get to medical appointments in the <br />local area.
