I <br />BACKGROUND AND COUNTY ENVIRONMENTAL REPORT <br />CHANGE OF ZONE REQUEST (RS-10 TO CN-20) <br />TMK: (3)1- S- 003:037" <br />A. SUBJECT REQUEST <br />1. Details of Proposed Use/Development <br />a. PROJECT DESCRIPTION <br />The applicant, Aguinaldo 4,, LLCM, wishes to change the zoning of this parcel from <br />Residential-10,000 square feet (RS-10) to Commercial Village-20,000 square <br />feet (CN-20). <br />The applicant is proposing to construct a dual purpose medical facility, providing <br />a kidney dialysis treatment center and geriatric services. <br />Within the dialysis center there are proposed patient and professional areas. The <br />patient treatment area is 8' by 8', located in an open area with 16 proposed chairs. <br />The center will have individual televisions at each chair, or internet,, as most dialysis <br />patients do not talk to each other and most want to be left alone for the treatment. <br />Also included will be 4 'Isolation rooms for patients who are very ill or contagious. The <br />center will also include planned nurses' stations; a separate home hemo/PD office <br />and training room, several offices for nurses, social workers, d irector and the <br />nutritionist, a staff break room and storage. The center will have a "water room" <br />that holds the equipment for preparing the water for dialysate. Theis room will be 16' <br />x 16' and needs a curb around it and floor drains if something happens to leak. <br />Geriatric physicians specialize in managing patients with complicated medical <br />diagnoses to help them maintain health and quality of life as they age. The emphasis <br />of the clinic is that it will allow older citizens to live in their homes 'independently, as <br />long as possible. Having a care center in Pahoa will enable seniors to have regular <br />care without the necessity of driving to Hilo; in many cases, having to be taken, to <br />Hilo. There will be a unique affiliation allowing physician specialists to offer <br />comprehensive care for older patients in an environment that thrives on <br />collaboration between physicians and social workers of many different specialties <br />and from different areas in Hawaii. <br />b. OBJECTIVES <br />The proposed use would provide residents of Puna with an alternative to <br />traveling to Hilo for these medical services. There is only one dialysis center in <br />Hilo. As of 2010, the population of the district of Puna over 60 years of age was <br />18.50. With Puna the fastest growing district, there will be a critical need for <br />aging services in the future. <br />c. SIZE <br />The subject parcel is 1.545 acres, 67,300 square feet. <br />
