2.0EXECUTIVE SUMMARY <br />Lehua Environmental Inc. (LEI) has completed this supplemental hazardous materials survey for the <br />County of Hawaii Keaau Fire Station located at 16-579 Old Volcano Road in Keaau, Hawaii (Subject <br />Site). This report supplements the Previous Report conducted by EKNA Service, Inc. On September <br />22, 2017, LEI personnel performed site reconnaissance to identify and inventory suspect asbestos- <br />containing material (ACM), lead painted surfaces, mercury-containing lamps and PCB-containing light <br />ballasts not previously identified at the Subject Site. <br />Summary of ACM Survey <br />The 1” x 1” ceramic floor tiles and associated grout and leveling compound were identified to be non- <br />ACM by laboratory analysis. No ACM was identified during this supplemental hazardous materials <br />survey at the Subject Site. <br />Table 1 located in Appendix I summarizes this result. <br />Summary of Lead Paint Survey <br />One (1) sampled painted surface was identified to have a lead paint concentration greater than or equal <br />to 5,000 mg/kgand is considered to be Lead-Based Paint (LBP). The LBP was identified was the beige <br />metal conduit pipe located in Room 108 of the Subject Site.
