3.0INTRODUCTION/PURPOSE <br />The purpose of this supplemental limited hazardous materialssurveywas to investigate the <br />Subject Sitefor the presence of hazardous materials not previously identified in the Previous <br />Reportfor the Subject Site.LEI identified the hazardous materials not previously identified in <br />the Previous Report to be suspectasbestos-containing materials (ACM). Specifically, LEI <br />completed the following tasks: <br />Reviewedthe 2008Hazardous Material Survey Reportfor the Subject Sitecompleted by <br />EKNA Services, Inc. (Previous Report); <br />Performed site reconnaissanceand visually inspected the areas ofthe Subject Site; <br />Collected samples of suspectACMfrom the Subject Site; <br />Submitted thesamples of suspectACMto NVL Laboratories, Inc.for analysis;and <br />Prepared this report documenting the field activitiesand the results of the survey, <br />including analytical results, photographs, recommendationsand maps. <br />Supplemental Limited Hazardous Materials Survey March 23, 2015 <br />County of Hawaii, Civil Defense Warehouse <br />630 E.
