4.0METHODOLOGY <br />4.1AsbestosSurvey <br />LEIpersonnel collected a total of three(3) samples of suspectbuilding materials for asbestos <br />analysis. All of the suspectACM samples were collected in general accordance with EPA <br />guidelines and recommendations. <br />The suspectACMswere wetted with amended water before sample collection. A small piece <br />was then carefully cut out and placed into a labeled re-sealable plastic bag. Thesampling <br />equipment was cleaned between each sample collection to avoid cross-contamination between <br />samples. The approximate quantity of each suspectACM was noted. Sample locations were <br />randomly selected in accordance with EPA protocols and recommendations. <br />All samples were properly logged and recorded following strict chain of custody procedure and <br />submitted to NVLLaboratories, Inc.in Seattle, Washingtonfor analysis by polarized light <br />microscopy in accordance with EPA Method 600/R-93/116. NVLLaboratories, Inc.is <br />accredited for bulk asbestos analysis through successful participation in the National Voluntary <br />Lab Accreditation Program (NVLAP). <br />4.2Lead Paint Survey <br />LEI personnel inspected the Subject Site for previously unidentified lead painted materials. <br />None were observed; therefore no paint samples were collected. <br />Supplemental Limited Hazardous Materials Survey March 23, 2015 <br />County of Hawaii, Civil Defense Warehouse <br />630 E.
