ofcommunity input atthe various public hearings, In additon to these consultations, the OHCD atso <br />informed each Council Member representing all districts in the County of Hawaii of the CP process and <br />provided them with an opportunity to inform the County of any projects that would meet the draft <br />priorities vvithin their district. <br />Describe coordinaton with the Continuum of Care and efforts to address the needs of <br />homeless persons (particularly chronicafly homeless individuals and families, families with <br />children, veterans, and unaccompanied youth) and persons at risk of homelessness. <br />The State of Hawaii is ibe primaryagency that provkies homeless funds to homeless service <br />providers. The County actively participates n the Bridging the Gap (Neighbor island counties) and <br />Statewide (consisting of the State and Bridging the Gap) Continuum of Care, These groups focus on <br />approaches to ending homelessness. The County is also a cornponent of the Interagency Council 00 <br />Homelessness a conglomerate of Government and service providers that seek solutions to address <br />statewide chronic homelessness through mainstream re -programming. Participation in the Continuum <br />also allows the County to acquire other HUD funds for activities that address homeless needs within the <br />CountyThe County will continue to participate in all levels of collaboration in a proactive effort to <br />address the needs to its homeless oonornunity. <br />Describe consultation with the Continuum(s) of Care that serves the jurisdiction's area in <br />determining how to allocate ESGfunds, develop performance standards for and evaluate <br />outcomes of pr-ects and activities assisted by ESG funds, and develop funding, policies and <br />procedures forthe operation and administratior, of HM1S <br />The County of Hawaii is not a recip ent of Emergency Solution Grant Funds. The State of Hawait <br />allocates these funds to eligible homeless providers., <br />2.
