8 � Annual Report, 2018 <br />SERVING FRAIL SENIORS AGING IN PLACE <br />HCOA solicited the interest of over 15 community agencies to provide wrap-around <br />services helping frail seniors and their families divert long-term institutional placement. <br />Agencies were selected through a competitive Request for Proposals process and then <br />monitored quarterly for the appropriate delivery of services. Hawai'i Island providers <br />contracted by HCOA to provide community-based services include, but are not limited to <br />Services for Seniors, Kona Day Care, Hawaii Island Adult Day Care, Ho'onani Place, <br />Mastercare, Metrocare, and Seniors Helping Seniors. <br />KUPUNA (ELDER) CARE PROGRAM <br />The Kupuna Care Case Management program provided assistance to 439 clients, families, and <br />caregivers in identifying needs, exploring options, and mobilizing informal and formal supports <br />to achieve the highest possible level of client independence. Case Management assistance <br />includes assessing needs, developing care plans, and coordinating provision of services among <br />Hawai'i County providers. This program provided the following services in FY 2017-18: <br />Personal Care, homemaker, and chore services utilized for a total of 22,888 hours of <br />assistance to seniors unable to perform daily activities (such as eating, dressing or <br />bathing) or instrumental activities of daily living such as shopping or light housework. <br />Adult day care services were provided for a total of 10,075 hours of care for dependent adults <br />in a supervised, protective group setting during some portion of a twenty -four-hour day thus <br />allowing socialization for our kupuna and rest and recuperation for the caregiver(s). <br />Case Management Services were provided to 439 seniors and their caregivers for a <br />total of 8181 hours of assistance in assessing needs, developing care plans, and <br />arranging services. <br />FIOME DELIVERED MEALS FT,0C_'T'RAM <br />The Home Delivered Meals Program served 84,209 meals to 577 frail home -bound <br />seniors. This In -Home service provides nutritious meals, and daily assurance checks. <br />Seniors gather at an adult day care <br />center, enjoying a meal and <br />conversation together while their <br />primary caregiver is on respite. <br />
