provided 99,780 hours of service to 3,628 <br />individuals at 203 volunteer stations island - <br />wide. <br />SENIOR TRAINING AND EMPLOYMENT <br />PROGRAM (STEP) <br />Each year HCOA works closely with the <br />Hawaii County STEP program to ensure <br />that older adults have opportunities for <br />returning to the work force. In FY2017, 22 <br />seniors were placed in supportive <br />employment programs with the intention of <br />securing stable employment after two <br />years. Two trainees were placed at the <br />East Hawaii ADRC. <br />TRANSPORTATION <br />In FY2017, Coordinated Services for the Elderly <br />(CSE) provided over 1000 seniors with <br />transportation to -and -from congregate meal <br />sites, medical and health related visits and <br />other community resource services. <br />HCOA SERVED <br />FRAIL SENIORS AGING IN PLACE <br />HCOA solicited the interest of over 15 community <br />agencies to provide wrap-around services and <br />supports helping frail seniors and their families <br />divert long-term institutional placement. Agencies <br />were selected through a competitive Request for <br />Proposals process and then monitored quarterly <br />for the appropriate delivery of services. Hawaii <br />Island providers contracted by HCOA to provide <br />community-based services include, but are not <br />limited to Services for Seniors, Kona Day Care, <br />Hawaii Island Adult Day Care, Ho`onani Place, <br />Mastercare, Metrocare, Altima Care, Seniors <br />Helping Seniors, Aloha Maiden, and Care <br />Resource Hawaii. <br />KUPUNA (Elder) CARE PROGRAM <br />In FY2017, the Kupuna Care Case <br />Management program provided assistance to <br />517 clients, families, and caregivers in <br />identifying needs, exploring options, and <br />mobilizing informal as well as formal supports to <br />achieve the highest possible level of client <br />independence. Case Management assistance <br />includes assessing needs, developing care <br />plans, and coordinating provision of services <br />among Hawaii County providers. The Kupuna <br />Care program provided the following services in <br />FY 2016-17: <br />Personal Care, Homemaker, and Chore <br />Services were utilized for a total of 16,048 hours <br />of assistance to seniors unable to perform daily <br />activities (such as eating, dressing or bathing) or <br />instrumental activities of daily living such as <br />shopping or light housework. <br />• Adult Day Care services were provided for a <br />total of 9,903 hours of care for dependent <br />adults in a supervised, protective group setting <br />during some portion of a twenty-four hour day <br />thus allowing socialization for the kupuna and <br />rest and recuperation for the caregiver(s). <br />• Case Management Services were provided <br />for a total of 7,687 hours of assistance in <br />assessing needs, developing care plans, and <br />arranging services for 517 seniors and their <br />caregivers. <br />HCOA staff Horace Farr (left) welcomes former HCOA <br />executives Jane Testa and Bill Takabe as distinguished <br />guests at the annual older American luncheon <br />HOME DELIVERED MEALS <br />In FY 2017, the Hawaii County Nutrition <br />Program served 70,775 meals to 478 frail <br />homebound seniors.
