These funds, derived from the U.S. Administration of Aging, will be used to <br /> continue their implementation of the CDSMP, which is an evidenced-based prevention <br /> program. The Office of Aging will coordinate the delivery of this program through an aging <br /> service provider organization. <br /> <br /> Enclosed is a bill for an ordinance amending the Operating Budget by appropriating the <br /> $28,348 into the Healthy Aging/Chronic Disease Self Management Program account. <br /> <br /> If there are any questions, please do not hesitate to call the Office of Aging. <br /> <br /> <br /> <br /> <br /> Nanc E.
