(Charter 10-16(h)(4)(A)) <br /> ❑ 2. IRS 501(c)(3) Determination Letter: Confirming the organization's 501(c)(3) status or <br /> that of an umbrella organization. (Charter 10-16(h)(4)(13)) <br /> ❑ 3. Bylaws and Mission Statement: Copy of the organization's bylaws and mission <br /> statement. (Charter 10-16(h)(4)(C)) <br /> ❑ 4. Detailed Business Plan: Information provided in the application contains a business <br /> plan detailing the project,including timeframes,costs,activities, and any additional <br /> information requested by the Department of Finance. (Charter 10-16 (h)(4)(D)) <br /> ❑ 5. Signed Agreement for Reporting: Submitted a signed agreement to file a report within <br /> one year after receiving funds or 30 days following project completion.The applicant <br /> acknowledges that the report should include project details, expense receipts (proof of <br /> payment) and any other information requested by the Department of Finance. (Charter 10- <br /> 16(h)(4)(E)) <br /> ❑ 6.Agreement for Non-Salaried Officers and Board Members: Submitted a signed <br /> statement confirming that no officers,board members, or employees will receive a salary <br /> from the funds for general duties,with the exception of compensation for specific duties <br /> outlined in the business plan. (Charter 10-16(h)(6)) <br /> ❑ 7.
