In addition to the Title V1 complaint process at MTA, any person who believes she or he has <br />been aggrieved by an unlawful discriminatory practice under Title VI may file a complaint, which <br />must be filed within 180 days of the alleged occurrence to any of the following: <br />County of Hawaii <br />Department of Human Resources <br />Attention: EEO Officer/ADA Coordinator <br />25 Aupuni Street <br />Hilo, Hawaii 96720 <br />Phone No. (808) 961-8361 <br />External Entities: <br />Hawaii Department of Transportation <br />Civil Rights Office (OCR) <br />200 Rodgers Blvd. <br />Honolulu, Hawaii 96819 <br />Phone: (808) 831-7924 <br />Email: HDOT-TITLEVI@hawaii.go <br />Federal Transit Administration <br />Office of Civil Rights <br />Attention: Complaint Team <br />East Building, 5th Floor — TCR <br />1200 New Jersey Avenue, SE <br />Washington, DC 20590 <br />A court, Federal or State <br />If a complaint is filed with both MTA and an external entity, the external complaint will <br />supersede the MTA complaint and the MTA complaint procedures will be suspended until the <br />external entity produces findings. <br />MTA Complaints Process <br />Complaints that allege discrimination based on race, color, or national origin, while using any <br />Hele-On Bus services or programs, will be assigned a complaint number by MTA. <br />MTA will notify the County's Equal Opportunity Officer/ADA Coordinator of the complaint. <br />MTA will review the Title VI complaint and will provide appropriate assistance to complainant(s), <br />including those who have limited English proficiency (LEP). <br />MTA will begin investigation of a formal Title VI complaint within ten (10) working days of <br />receiving the complaint. <br />If additional information is needed from the Complainant, MTA will contact the complainant in <br />writing within fifteen (15) working days for additional information. If the complainant fails to <br />provide the requested information by the requested date, the complaint could be <br />administratively closed. <br />
