to 24 months of rental assistance for fair market rental housing. <br />Rapid Re -Housing: <br />To provide housing relocation and stabilization services and rental assistance to quickly move <br />households experiencing homelessness into stable, permanent housing. The County will support <br />homeless programs grant subrecipients that provide services to homeless individuals and families living <br />in the County to maintain or identify alternative permanent rental housing achieve housing <br />stability. Housing relocation and stabilization can include, but is not limited to, financial assistance <br />including moving related costs and utility assistance, housing search and placement and housing stability <br />case management. Households may also receive up to 24 months of rental assistance for fair market <br />rental housing. <br />Homeless Management Information System: <br />To support the expansion and data quality of the Homeless Management Information System (HM15) for <br />the Bridging the Gap, Continuum of Care by convening and providing technical assistance to homeless <br />assistance providers. in coordination with the Bridging the Gap, the County will support the <br />improvement of homeless data collection to improve community outcomes for homeless individuals. <br />Annual Action Plan <br />20121 <br />OMB control No: 2505-0317 {exp. 09/W <br />0 <br />a] <br />
