T
50. In 2,000 characters or less, describe the measurable outcomes you aim to achieve with these
funds.
Measurable outcomes include: (1) enhanced community engagement through at least three large-scale wellness and
cultural events with minimum attendance targets, fostering local participation and connection; (2) increased access to
holistic health services by supporting collaborative clinics and wellness offerings, documented by a specific number of
clinic visits or client sessions; (3) strengthened local economic activity by supporting vendors and small businesses at
these events, measured by the number of vendors served and sales impact reports; (4) improved outreach and
educational impact, measured by the number of workshops, presentations, and informational materials distributed,
reaching targeted population segments; and (5) documented volunteer engagement and training completion rates to
build sustainable community leadership and event management capacity. These outcomes will be tracked through
attendance logs, participant surveys, vendor feedback, and progress reports to ensure transparent and effective use of
the funds to boost community wellness, cultural vitality, and sustainable local development.
