<br /> Nov 29 U3 02:16p p,3 <br /> RULES AND REGULATIONS OF HOOKENA WATER ASSOCIATION <br /> The following rules and regulations are made and approved by the Hookena Water <br /> Association (the "Association") and no official, employee or agent ofthe Association has <br /> any power or authority to alter or change the rules and regulations without written <br /> consent of all parties. The Associafion is not a Public Utility as defined by the Hawaii <br /> Revised Statutes, and is, and will, only supply water to approved members, and not to the <br /> public in general. <br /> 1) APPLICATION FOR CONNECTION <br /> Connection to the mains ofthe Association will be made, or remain in effect. after <br /> January 1st 2004, only on the written application of the owner of the premises to be <br /> supplied with water, or the owner's authorized agent, after the applicant agrees to comply <br /> with the rates, rules and regulations of the Association, and after the approval of the <br /> application by the Association endorsed on the application. The size of service lines, <br /> valves, meters and other fittings, fixtures or appliances necessary to render the service for <br /> which application is made shall be determined by the Association. <br /> 2) APPLICATION FOR SERVICE <br /> On application of the owner or any authorized agent or any tenant of the owner, water <br /> will be famished through existing connections, or, if necessary to conform to the <br /> Association's requirements for galvanized pipes and/or for an approved meter, on <br /> application of the owner, through new connections, after the owner agrees to comply with <br /> the rates, Hiles and regulations of the Association, and after the approval ofthe <br /> application, and installations, by the Association endorsed on such application. <br /> 3) CHANGE IN OWNERSHIP, TENANCY OR SERVICE <br /> A new application must be made and approved by the Association on any change in <br /> ownership of property, or in tenancy or in the service, as described in the original <br /> application, and the Association may discontinue the water supply until such new <br /> application is made and approved. No refund will rendered of dues or fees paid. <br /> 4) PAYMENTS <br /> At the time of making application for conncction or service, the applicant must make <br /> payment to the Association of the fees, deposits or rates, as the case may be, fixed by the <br /> Association for the service requested. If the application is rejected for any reason, the <br /> fees, deposits or rates will be refunded in full within 30 days. <br /> 5) CONNECTIONS <br /> A. The Association will furnish and maintain at its own cost all necessazy fittings, pipes <br /> and appliances, necessary for delivery ofwater to the consumer's pipeline at a point at or <br /> <br />
