Attachment I <br />2. HCPD shall ensure that the BTC will adhere to and complete all tasks outlined in the position <br />description detailed in Exhibit "A" attached hereto and made a part hereof, prior to the end of <br />this Agreement; <br />3. HCPD shall develop, coordinate and implement acounty-wide Balanced Transportation <br />Plan/Program that: <br />a. Integrates that Department of Health ("DOH"), Healthy Hawaii Initiative ("HHI") <br />concepts of healthy living and active communities; and <br />b. Shall be developed in collaboration with DOH HHI who shall serve as a partner and <br />resource. <br />4. HCPD shall develop a timeline to include, but not be limited to: <br />a. Listing of outcomes for the Agreement period; and <br />b. Timeframes, both estimated and actual, for the completion of each outcome. <br />5. HCPD shall work in collaboration with the DOH HHI and HHI Evaluation Team ("HHIET") <br />to: <br />a. Develop an evaluation tool that will assess the effectiveness of the BTC position; and <br />b. Track and monitor the completion of designated outcomes. <br />6. HCPD shall provide quarterly progress reports utilizing HHIET evaluation templates; and <br />7. HCPD shall provide a frnal report to DOH HHI upon completion of the Agreement. <br />DOH REF. NO. HHI-08-06 Z <br />
