Attachment 3 <br />COMPENSATION AND PAYMENT SCHEDULE <br />In fitll consideration for the services performed by the HCPD under this MOA, DOH HHI <br />agrees, subject to allotments to be made by the Director of Finance, State of Hawaii, pursuant to <br />Chapter 37, Hawaii Revised Statutes, and subject to the availability of tobacco settlement funds, <br />to pay to the HCPD a total sum of money not to exceed SEVENTY-TWO THOUSAND, ONE <br />HUNDRED AND THIRTY-EIGHT AND NO/100 DOLLARS ($72,138.00}, in accordance with <br />and subject to the following: <br />a. Payment shall be made upon aone-time submission by the HCPD on an invoice <br />in triplicate, for the services to be provided in accordance with Attachment 1, <br />"Scopes of Services," and in accordance with the costs identifed in the Position <br />Description, attached hereto as Exhibit "A" and made, a part hereof. <br />b. The HCPD shall submit to the DOH HHI quarterly progress reports providing <br />updates and accomplishments for the BTC position. The level of detail required <br />on the progress reports shall be mutually agreed to by the HCPD and DOH HHI. <br />c. Completion of this Agreement shall include submission and acceptance of all <br />reports and other materials to be submitted by the HCPD to the DOH HHI, <br />resolution of all discrepancies in expenditures or performance of services, and <br />completion of all o#her outstanding matters under this Agreement. <br />DOH REF.
