<br />Compensation
<br />OHA shall provide TWENTY-FIVE THOUSAND AND NO/100 DOLLARS
<br />($25,000.00), subject to the availability of funds, payable to HCPD as follows:
<br />A. An INITIAL payment of THIRTEEN THOUSAND AND NO/100
<br />DOLLARS ($13,000.00) upon execution of the Ageement and submission of
<br />a copy of the press release or proof thereof acknowledging OHA's funding
<br />support.
<br />B. A SECOND (2"d) payment of FIVE THOUSAND AND NO/100 DOLLARS
<br />($5,000.00) payable upon submission and approval of progress activity and
<br />financial reports, satisfactory to OHA, for the period of May 1, 2009 through
<br />June 30, 2009, due on or before July 31, 2009.
<br />C. A THIRD (3`a) payment of FIVE THOUSAND AND NO/100 DOLLARS
<br />($5,000.00) payable upon submission and approval of progress activity and
<br />financial reports, satisfactory to OHA, for the period of July 1, 2009 through
<br />October 31, 2009, due on or before November 30, 2009.
<br />D. A FOURTH (4th) payment of ONE THOUSAND FIVE HUNDRED AND
<br />NO/100 DOLLARS ($1,500.00) payable upon submission and approval of
<br />progress activity and financial reports, satisfactory to OHA, for the period of
<br />November 1, 2009 through January 31, 2010, due on or before February 28,
<br />2010.
<br />E. A FIFTH (5th) and final payment of FIVE HUNDRED AND NO/100
<br />DOLLARS ($500.00) payable upon submission and approval of final activity
<br />and financial reports, satisfactory to OHA, to include the period of February 1,
<br />2010 through April 30, 2010, due on or before May 31, 2010.
<br />Reporting Requirements
<br />A. Quarterly and Final Reports to OHA. HCPD shall submit quarterly and final
<br />reports to OHA. HCPD will be assigned to an OHA gant monitor who will
<br />provide technical assistance for the reports and address progam concerns as
<br />needed. The OHA grant monitor will also conduct a site visit during the contract
<br />award period.. In addition, HCPD shall submit to OHA on or before May 31,
<br />2010 a final report, satisfactory to OHA, which shall contain a final expenditure
<br />report together with unexpended balances of funds. HCPD shall submit quarterly
<br />and final reports based on due dates.
