Appeals submitted after the time limits prescribed in <br /> this section will be considered untamely• <br /> Petition of appeals shall be filed with the fire chief, Hawaii Coun Fire <br /> Department 777 Kilauea Avenue. Mall Lane Room 6, Hilo. Hawaii 96720. <br /> <br /> ~ The petition of appeal shall contain the following information: <br /> Petitioner's name, address, and telephone number. <br /> The issue on appeal. <br /> A statement of the relevant facts. <br /> The remedy sought including the rationale and legal basis in support of the <br /> requested remedy. <br /> (5 Petitioner's signature and the date thepetition is submitted to the fire chief, <br /> of the County of Hawaii. <br /> Section 14-34. Hearin. <br /> ~ Within ten calendar days from the filing_of a petition of appeal the fire chief or a <br /> designated hearing officer shall notify the petitioner in writing as to the date time <br /> and place of the hearing_b~rggistf;red or certified mail with return receipt <br /> rectuested The petitioner shall be given written notice of hearing at least f fifteen <br /> days prior to the hearing. <br /> The notice of hearin¢ and hearing shall be in conformity with the provisions of <br /> chapter 91 Hawaii Revised Stattites relating to contested cases. <br /> Section 14-35.
