AGING <br /> AGING <br /> • <br /> Program Objectives (continued) <br /> 3. Community Planning and Development — Partnerships <br /> Healthy Aging Initiative <br /> a. Increase by 20% the number of chronic disease self management workshops <br /> offered in Hawai`i County. The 20% increase would be based on the number of <br /> workshops completed for the fiscal year ending June 2009. The intent is to <br /> embed a health related evidence -based program in Hawai`i County. <br /> b. Support and coordinate an evidence -based community program where at least <br /> 75% of participants surveyed will indicate that the evidence -based community <br /> program improved or maintained their health status. <br /> 4. Education/Training <br /> Support and coordinate a minimum of two (2) workshops and/or educational seminars <br /> for Aging and Disability Resource Center (ADRC) clients and/or service providers <br /> that reflect identified needs of clients and providers. <br /> 5. Home Modification <br /> Enable participants to live safer within their own home and help to correct safety <br /> deficits. <br /> • At least 75% of the clients served will indicate that in -home safety items <br /> and/or adaptations provided allowed them to remain in their homes by <br /> correcting safety deficits. <br /> • At least 75% of the clients served will indicate that the safety items provided <br /> led to reduced falls in the home. <br /> Services Contracted to Private /State Agencies <br /> 1. Kupuna Care Program <br /> Case Management and Home and Community Based Services <br /> Kupuna Care Program services include case management, adult day care, personal <br /> care, home - delivered meals, heavy chore, homemaker, assisted transportation, and <br /> Kupuna Care transportation. <br /> Provide older adults with home and/or community -based services through the Kupuna <br /> Care Program (KC). <br /> • At least 75% of Kupuna Care Program clients will remain in their homes at <br /> least three months. <br /> • At least 75% of Kupuna Care Program clients surveyed will report their care <br /> needs are being met by the program. <br /> 5 <br />
