Legal Services <br /> On an annual basis, at least 85% of older adults who request information about legal <br /> advice, counseling, and representation will be linked to legal resources and at least <br /> 70% of the older adults serviced and surveyed during the year will improve their <br /> knowledge of entitlement programs, the legal process, and /or individual rights. <br /> 3. Public Education <br /> Offer the general public a monthly publication called the Silver Bulletin which <br /> provides information about services and benefits available to older individuals and <br /> their caregivers where at least 75% of clients served during the year will report <br /> satisfaction with the information content, quality and relevance of the newsletter as <br /> well as usefulness of the newsletter regarding benefits and programs around Hawai`i <br /> County. <br /> 4. Transportation. <br /> Provide transportation service to congregate meals participants in order for them to <br /> receive nutritious meals and participate in site activities that enhances physical and <br /> social well- being. <br /> a. The percentage of participants identified as non - nutritional risk at the <br /> beginning of the program year shall be maintained at the same or higher levels <br /> by the end of the program year. <br /> b. The percentage of participants identified as high nutritional risk at the <br /> beginning of the program year shall be maintained at the same or lower levels <br /> by the end of the program year. <br /> Contracts to the Elderly Activities Division, Department of Parks & Recreation. <br /> Funds provided to the Elderly Activities Division are deposited directly to its respective <br /> accounts. Elderly Activities Division provides the following contracted services and <br /> reflects its own program objectives. <br /> • Congregate Meals <br /> • Training and Employment <br /> • Home - Delivered Meals <br /> • Nutrition Education <br /> • Outreach <br /> • Transportation <br /> • Volunteer Services <br /> 6 <br />
