AGING <br /> AGING <br /> c. Provide access to all districts of the island, trained individuals to assess caregiver <br /> needs, provide information and assistance that link caregivers to needed services <br /> and provide follow -up on an as- needed basis. At least 80% of requests for <br /> information and /or services will have immediate (within 7 days) contact with an <br /> assessor to begin the assessment process and identify caregiver needs. <br /> Five training sessions and /or conferences were held during the fiscal year: <br /> 12/1/08 Know Your Options (Grandparents Raising Grandchildren) <br /> Coordinated with Legal Aid Society Hilo <br /> 12/3/08 Know Your Options (Grandparents Raising Grandchildren) <br /> Coordinated with Legal Aid Society Kona <br /> 1/27/09 Carousel of Care Caregivers Conference Kona <br /> 5/14/09 Basic Caregiving Skills Workshop at Hospice of Kona <br /> 6/16/09 Challenges of Caregiving Workshop in Pahoa <br /> All participants surveyed reported they were satisfied with the content, <br /> usefulness and effectiveness of the information covered at the training <br /> sessions and would like more training sessions in the future. The training <br /> sessions and conferences increased their knowledge of resources available for <br /> Grandparents Raising Grandchildren and increased their knowledge of in- <br /> home caregiving skills and essential resources for caring for loved ones in the <br /> home. <br /> 3. Community Planning and Development — Partnerships Healthy Aging Initiative <br /> Establish a Healthy Aging Partnership with public and private agencies and elderly <br /> consumers. <br /> Support and coordinate a chronic disease self management program where at least 75% <br /> of participants surveyed will indicate that the self management program improved or <br /> maintained their health status. <br /> As of September 1, 2008, a Hawaii County Healthy Aging Partnership was <br /> established to implement and embed an evidence -based self management <br /> program for older individuals. With Hawaii County Office of Aging providing <br /> leadership, partners include Alu Like, Inc., Bay Clinic, Inc., West Hawaii <br /> Community Health Center, Services for Seniors, Kau Rural Health Community <br /> Center, and YMCA - Waiakea Settlement. The evidence -based program is <br /> Stanford's Chronic Disease Self Management Program (CDSMP). It is a 2 -1/2 <br /> hour once a week for 6 weeks program with a 6 month followup. <br /> Final Status Report on Program Objectives for FY 2008 -09 4 <br />
