AGING <br /> AGING <br /> During the 2009 fiscal year, self management workshops were conducted for <br /> Chronic Conditions, Diabetes and Arthritis serving 82 individuals. All <br /> participants that were surveyed six months after attending the workshops <br /> indicated improved or maintained health status. All data sheets were sent to <br /> Honolulu to be compiled with the other counties for a statewide report to the <br /> Administration on Aging and Stanford School of Medicine. <br /> 4. Database Development and Maintenance <br /> a. Collaborate with private and public sectors to provide training and technical <br /> assistance to HCOA service providers on data collection and reporting. At least <br /> one training meeting will be conducted during the first half of the program year. <br /> Due to the planned move into the new facility at 1055 Kinoole Street, HCOA <br /> data staff met with Service Provider staff individually to provide technical <br /> assistance, and training to new staff. Then during the second six months of <br /> FY 2009, HCOA data staff person moved HCOA data equipment out of Hilo <br /> Lagoon Building and into the ADRC Building. In addition to reactivating <br /> the system, the HCOA data staff person continued to meet with service <br /> providers on an individual as needed basis. <br /> b. Respond to and complete all data/information requests in a timely manner. 90% <br /> of data/information requests will be completed within 10 working days of receipt <br /> of request. <br /> HCOA data staff responded to five data requests during the 2009 fiscal year. <br /> Three requests were from the State Executive Office on Aging, requesting <br /> demographic data on consumers served in specific services in the previous <br /> Fiscal Year. One request was made by the editor of the Silver Bulletin on the <br /> number of seniors who were born in the Chinese Year of the Ox for a feature <br /> story. The final request was received from the Food Basket Inc. Food <br /> Basket Inc. requested a report on the number of seniors HCOA served who <br /> may have been eligible to participate in the island -wide senior produce <br /> distribution program. <br /> Final Status Report on Program Objectives for FY 2008-09 5 <br />
