BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): <br />The Hawaii County Office of Aging recently partnered with the State Executive Office on Aging in applying <br />for the following federal grants: 1) CMS Person Centered Hospital Discharge Planning grant to develop a <br />model to put the patient and caregiver(s) at the center of the discharge planning process and focus on <br />discharging patients home with community -based services, via the Aging and Disability Resource Center, <br />thereby reducing the number of patients retained in acute care beds past the point of clinical discharge, and . <br />reduce the number of default discharges from acute care units to nursing facilities. 2) ADRC — Establish new <br />or significantly strengthen existing ADRC programs, develop a 5 year operational plan and budget. <br />We have received a contract for the federal funding of these 2 pilot programs over a three year period. In <br />addition, the state is offering the counties an additional $50,000 in state general funds for ADRC operations. <br />The total combined contract is for $267,922.00 <br />However, in order to receive the additional state funding of $50,000, the contract must be processed and <br />returned to the State no later than June 10, 2010.
